Please use this form to submit a parent note for your student’s absence(s). Each student may have up to five absences excused with a parent note per school year. After all five parent notes have been used, medical documentation must be submitted for any additional absences to be considered excused. All documentation should be provided to the school within five days of the student’s return following an absence.
